This Privacy Policy describes the LGU-Ormoc's policies and procedures on the collection, use and disclosure of Your information when You use the Service and tells You about Your privacy rights and how the law protects You.
We use Your Personal data to provide and improve the Service. By using the Service, You agree to the collection and use of information in accordance with this Privacy Policy. This Privacy Policy is maintained by the LGU Ormoc- IT Division.
This Privacy Policy explains what information is collected, processed, and shared; the reasons for doing so; and Your rights with regard to your information. We process Your information pursuant to Republic Act No. 11332 (the "Mandatory Reporting of Notifiable Diseases and Health Events of Public Health Concern Act") and in accordance with Republic Act No. 10173 (the "Data Privacy Act").
We may amend this Privacy Notice from time to time. When certain changes require your consent, we shall communicate the changes to You and ask for Your consent. Otherwise, changes take effect immediately upon posting.
Definitions
For the purposes of this Privacy Policy:
- You/r means the individual accessing or using the Service, or the company, or other legal entity on behalf of which such individual is accessing or using the Service, as applicable.
- We/ Our/ LGU-ORMOC refers to the City Government of Ormoc acting through the LGU Ormoc- IT Division.
- Service refers to the Safe Ormoc V2 accessible from ormoc.gov.ph.
- Personal Data is any information that relates to an identified or identifiable individual.

While using Our Service, We may ask You to provide the service with certain personally identifiable information that can be used to contact or identify You. Personally identifiable information may include, but is not limited to:
- First name, middle name and last name
- Phone number
- Address
- Date of Birth
- Email Address
When you create an account with SafeOrmoc V2 on behalf of another, and you provide information, we will construe that You have obtained the necessary consent from them to both the disclosure and the processing of his/ her personal information in accordance with our policy.
WHY WE COLLECT YOUR INFORMATION
Use of Your Personal Data
We may use Personal Data for the following purposes:
- To generate unique QR Code as identification.
- For contact tracing purposes as an APOR, close contact, suspect/ probable/ confirmed COVID-19 case.
- To contact You: by email, telephone calls, SMS, or other equivalent forms of electronic communication, if ever You have a close contact/encounter with a person suspected with COVID-19.
- For all other concerns related to COVID-19.
Retention of Your Personal Data
We will retain Your Personal Data only for as long as is necessary for the purposes of contact tracing ONLY.
Transfer of Your Personal Data
We will take all steps reasonably necessary to ensure that Your data is treated securely and in accordance with this Privacy Policy and no transfer of Your Personal Data will take place to an organization or a country unless there are adequate controls in place including the security of Your data and other personal information.
Disclosure of Your Personal Data
Contact Tracing
As an APOR or if ever You become a close contact, or a COVID- 19 suspect/ probable/ confirmed individuals, We will then produce a report to be used by the Contact Tracing Team to be able to reach You and notify You.
